What is the best way to motivate oneself working out after a long break?

We often have motivational issues when going to the gym or other healthy physical activities. There will come a time when you just don't want to go. And if you've taken a break, the longer the break, the harder it will be to pick up the healthy habit again. How does one build again, and keep the motivation to continue taking care of their health?
